Hey there. Yes we did! We made an appointment within 10 days of our wedding (6/14 & we married on 6/22) and our doctor printed the following letter on her medical letterhead, signed and dated:

Affidavit of Medical Requirement

I, Christina Bailey, MD, do hereby affirm that Ashley Lastname and Charles Lastname have met the medical requirements for marriage in Our City, California, Riverside County, where they reside. As of the date of this letter, there are no medical requirements for the State of California.

If you have separate Doctors, you simply need a letter for each of you.

If you have any other license questions, I am happy to help. This was definitely something I was quite stressed about, but I ended up being well prepared and had no issues upon arrival!
